Acceptable Forms of ID for 1583 – Mailbox Rental Application

 

In order to provide you with a mailbox, we must obtain a copy of two of the following forms of identification:

 

Ø      Valid Driver License

Ø      State-issued, non-driver license ID card

Ø      Armed Forces ID card

Ø      Government ID card

Ø      University ID card

Ø      Recognized Corporate ID card

Ø      Passport

Ø      Alien Registration Card

Ø      Certificate of Naturalization

Ø      Current Lease, Mortgage or Deed

Ø      Voter Registration Card

Ø      Home or Vehicle Insurance Policy

Ø      Vehicle Registration

Ø      Utility Bill

 

The identification presented must confirm that the applicant is who he or she claims to be.  By verifying identification, the Postal Service will protect against delivering mail without verifiable consent of the actual addressee.

 

Instructions for Form 1583

 

Box 1: The date the 1583 is executed

 

Box 2: This is the place to list every name, or business name that will be receiving mail at this address. If husband and wife, both names go here. If there is a long list of names, AKA's, company names, etc., they all go here, attaching a separate sheet if necessary with all of the names.

 

Box 3: This is NETSEND.US’s mailing address plus the PMB designation.

Do not put, or provide, a “P.O. Box” for this address.  It is not a “P.O. Box”.

Box 5: You sign here to authorize NETSEND.US to accept Restricted Delivery items. This does not mean regular Certified, Insured, or Registered Mail – we must accept all of these items. Restricted Delivery is a separate, more expensive service that mailers use to have only the addressee sign for a mailpiece.

 

Box 6: Name of applicant. This is the name that must match the ID asked for in box 9. Again, if there is more than one name, list them all here. For expediency sake, just mark "Same as Box 2".

 

Box 7: Home address and phone. This must match the ID shown in box 9. NOTE – This cannot be NETSEND.US’s address.

 

Box 8: Two forms of ID, one of which must contain a photograph of the applicant, are to be logged here by NETSEND.US. We will photocopy these IDs and keep them on file.

 

Box 9: If the box is to be used for business purposes, once again list the business name(s) here. If not, mark N/A.

 

Box 10: This is the business address and telephone number of the applicant - only needed if they are using the box for business purposes and have checked off box 5. If not, mark N/A.

 

Box 11: If using the box for business purposes, write in the type of business. If not, mark N/A.

 

Box 12: If the applicant is using the box for business purposes, list every individual whose name will appear on incoming mail. Log their ID onto the back of the 1583, if necessary. Just because they list someone here does not authorize NETSEND.US to receive mail for that individual - they must also provide identification to NETSEND.US in person –or- by notarized submission if they can't appear in person.

 

Box 13: If the box is being used for business purposes, and it is a corporation, then list the name and addresses of its officers

 

Box 14: If the box is being used for business purposes, then list the county, state, and date of registration of that business name.

 

Box 15: This is where you sign, or, if you can't physically appear before NETSEND.US, you must have a notary public authenticate the form. DO NOT pre-sign the form – be sure it's correct and complete before you sign it.

NETSEND.US Mailbox Rental Agreement

This Agreement made on this day _________________ between _______________________________, hereinafter referred to as "Applicant", and NETSEND.US, hereinafter referred to as "Mail Service", shall be governed by these terms to which each party agrees:

1. By completing this form and USPS Form 1583, a copy of which will be made available to the United States Postal Service, applicant appoints Mail Service as agent for the receipt for a period not to exceed that for which rent has been paid in advance.  Applicant will pickup mail at least once a month or make other suitable arrangements, in advance, with Mail Service.  Mail Service will provide a lockbox key to applicant who may obtain his mail during the business hours posted by Mail Service. Should applicant appoint another person or organization, Mail service shall presume that possession of a key is evidence of authority to collect mail.

2. The key provided to applicant shall require a non-refundable cash deposit of $10, and remains the property of Mail Service and shall not be duplicated or modified by applicant. Applicant understands that the relationship of the parties hereto is one of bailment and not landlord and tenant.

3. Once Mail Service has placed applicant's mail in the assigned lockbox, the mail shall be deemed to have been delivered, and Mail Service shall not be responsible for loss, theft or damage. Mail Service is not engaged in the delivery of mail and cannot be responsible for failure of the United States Postal Service to deliver mail or to deliver it in a timely fashion or undamaged condition.

4.  Applicant agrees to use services in accordance with Mail Service rules and in compliance with all U.S. Postal regulations, as well as local, state and federal statutes and regulations.  Failure to do so may result in cancellation of service without notice or refund.

5.  Information provided by applicant will be kept confidential and will not knowingly be disclosed without applicant's prior consent, except for law enforcement purposes, in which case Mail Service intends to cooperate fully.  Law enforcement is further clarified to include all city, county, state or federal agencies or their representatives.

6.  Mail will not be accepted for more than three (3) persons or organizations in a single lockbox and each must complete a USPS Form 1583 and provide photo identification.  If applicant consistently receives substantially more mail than can be placed in a single lockbox, Mail Service reserves the right to require applicant to rent a larger size box or one or more additional boxes. Businesses are required to use at least a “Medium” sized mailbox without exception.  Special circumstances, e.g. of parcels, may require assessment of additional fees.  An unusually high volume of mail will result in either a higher fee being charged, or termination of the mail receiving service.  Applicant further agrees that parcels delivered to this address for the applicant will be delivered by common carrier only, that no truck line deliveries will be made, that parcels will be retrieved within 24 hours after delivery, and that no hazardous or dangerous material will be delivered to applicant.  Failure to adhere to any of these parcel delivery stipulations will result in termination of service.

7.  Applicant agrees to protect, indemnify and hold harmless Mail Service from and against any and all claims, demands and causes of action of any nature whatsoever relative to use of Mail Service facilities or services, and any expense incurred in a defense of same shall be reimbursed by applicant.

8.  Should Mail Service commit or fail to commit any act which results in disruption of service and applicant thereby suffers a loss, Mail Service's liability shall be limited to not more than the rental fees paid by applicant for service not yet received.  Mail Service shall not be liable for incidental or consequential damages.

9.  Certified, registered, insured, or C.O.D. mail or parcels will be accepted by Mail Service only in those cases in which applicant specifically authorizes same by checking box six (6) of USPS Form 1583 and provides full, advance payment of C.O.D. charges.

10.  Mail Service fees are due and payable in advance and notice thereof will be placed in applicant's lockbox.  No other notice will be required.  Failure to pay Mail Service fees when due may result in a late fee of $15 or disruption, or cancellation, of services.  Mail Service does not prorate fees and does not provide refunds in the event of cancellation by applicant.

11.  Applicant shall use the address designation "PMB" to designate their address.  NO OTHER DESIGNATION IS VALID. The U.S. Postal Service may refuse to deliver any piece of mail that does not include the PMB designation.  Applicant is responsible for notifying correspondents of the above address.  The address to be used by applicant for the purpose of receiving mail is as follows:

12.  Upon termination of services by Mail Service or failure to pay rent in advance by applicant, Mail Service shall not make applicant's mail available without payment therefore.  Applicant understands that the United States Postal Service will not forward or return mail without payment, and will not accept a Change of Address.  At termination of service, applicant, if he wishes mail forwarded after that date, shall provide Mail Service with a forwarding address and pay the required fees.  In the event applicant fails to do this, Mail Service shall refuse any further mail and. in the case of mail already received handle such mail in accordance with USPS DMM DO42.2.6 regulations.

Termination Addendum
